DEA registration is separate from state licensure. Severe hepatic impairment reduces eszopiclone clearance, so the maximum recommended dose in that population is 2 mg. 1 A thyroid-stimulating hormone (TSH) level is sometimes checked to rule out hypothyroidism as a secondary cause of fatigue that patients may misattribute to insomnia.
A urine drug screen is not required by Texas law for Schedule IV prescriptions, though some telehealth platforms include it in their standard protocol to identify undisclosed CNS depressant use (opioids, benzodiazepines, alcohol metabolites). Patients with a prior history of substance use disorder may be asked to provide a baseline screen before the first prescription is issued.

Patients aged 65 or older should begin at 1 mg and generally not exceed 2 mg, consistent with the American Geriatrics Society Beers Criteria recommendation to minimize sedative-hypnotic use in older adults due to fall and cognitive impairment risk. 12
Eszopiclone should be taken on an empty stomach or after a light meal. High-fat meals slow absorption and delay the onset of sleep induction. 1
Prior Authorization in Texas: What Plans Require
Most commercial insurance plans in Texas require prior authorization before covering eszopiclone. The specific requirements vary by payer, but a standard PA request typically demands documentation of the insomnia diagnosis, evidence that non-pharmacologic therapy (CBT-I or sleep hygiene education) was attempted or is contraindicated, and confirmation that at least one preferred generic sedative-hypnotic was tried and failed or is clinically inappropriate. 13
Texas Medicaid (STAR and CHIP programs) does not cover Lunesta or generic eszopiclone for the insomnia indication. The Texas Vendor Drug Program formulary lists eszopiclone as non-covered under behavioral health benefits.
