What Is Affinity Health Plan and Which Members Does This Question Apply To?
Affinity Health Plan is a New York-based managed care organization that serves Medicaid, Child Health Plus, and Essential Plan members, primarily in the New York metropolitan area. Most Affinity members are enrolled through a government-sponsored program, which changes the coverage calculus significantly compared to commercial insurance. Because Medicaid drug coverage is governed by both federal and state formulary rules, the answer to whether Wegovy is covered depends heavily on which Affinity product a member holds.
Affinity's Core Product Lines
Affinity operates three main product lines: Medicaid Managed Care, Child Health Plus, and the Essential Plan. A small commercial block also exists for employer groups. Each product line follows a different formulary. Medicaid Managed Care plans in New York are required to cover drugs on the New York State Medicaid fee-for-service (FFS) formulary as a floor, but plans may add or restrict drugs beyond that floor.
Why Government Plans Complicate GLP-1 Access
Federal Medicaid statute has historically excluded coverage for "drugs used for anorexia, weight loss, or weight gain" under 42 U.S.C. § 1396r-8(d)(2). New York State has not enacted a broad waiver to override this exclusion for semaglutide. As a result, most Affinity Medicaid members cannot access Wegovy through their pharmacy benefit for obesity alone. The FDA approved Wegovy specifically for chronic weight management in adults with obesity or overweight with at least one weight-related condition on June 4, 2021 [1]. That approval does not automatically create Medicaid coverage.
Does New York Medicaid (and Affinity Medicaid) Cover Wegovy?
New York Medicaid's current formulary does not include semaglutide 2.4 mg (Wegovy) as a covered benefit for obesity or overweight. The statutory exclusion described above applies. Affinity Medicaid members seeking Wegovy for weight management are therefore not covered under the standard pharmacy benefit as of the date this article was last reviewed.
The Ozempic Pathway: Diabetes Diagnosis Changes Everything
Semaglutide 0.5 mg, 1 mg, and 2 mg (brand: Ozempic) is FDA-approved for type 2 diabetes and cardiovascular risk reduction [2]. New York Medicaid does cover Ozempic for members with a confirmed type 2 diabetes diagnosis. Affinity Medicaid members with type 2 diabetes may therefore access semaglutide through the diabetes indication, though at the lower Ozempic doses rather than the 2.4 mg obesity dose.
The SELECT trial (N=17,604) demonstrated that semaglutide 2.4 mg reduced major adverse cardiovascular events by 20% in people with established cardiovascular disease and overweight or obesity but without diabetes [3]. The FDA subsequently expanded Wegovy's label to include cardiovascular risk reduction in March 2024 [4]. Whether Affinity or New York Medicaid will cover Wegovy under this cardiovascular indication remains unresolved as of mid-2025, and members should verify current formulary status directly with Affinity's pharmacy services team at (800) 223-7242.
Essential Plan and Child Health Plus
The Essential Plan in New York follows a formulary established by the New York State Department of Health. Weight-loss drugs remain excluded. Child Health Plus covers pediatric members; semaglutide is not FDA-approved for patients younger than 12 years old for obesity, and it is generally not covered on these plans [5].
Prior Authorization Requirements for Wegovy on Commercial Affinity Plans
For the smaller cohort of Affinity employer-group commercial members, Wegovy may appear on the formulary, typically on a specialty tier. Prior authorization is required in virtually every case. The standard criteria align with the FDA-approved indication and with guidance from the American Association of Clinical Endocrinologists (AACE), which recommends pharmacotherapy as an adjunct to lifestyle intervention when BMI is 30 or above, or 27 or above with at least one obesity-related comorbidity such as type 2 diabetes, hypertension, or dyslipidemia [6].
Typical Prior Authorization Criteria
Affinity commercial plans generally require all of the following before approving Wegovy:
- Documented BMI of 30 or above, or BMI of 27 or above with at least one weight-related comorbidity
- Documentation of a structured diet and exercise program lasting at least 3 to 6 months with inadequate response
- Prescriber attestation that the member has no contraindications (personal or family history of medullary thyroid carcinoma or MEN2)
- In some cases, a documented trial and failure of at least one alternative anti-obesity medication
The 2023 American Heart Association/American College of Cardiology obesity guideline states: "Pharmacotherapy is an appropriate adjunct to intensive lifestyle intervention in patients who have obesity or overweight with weight-related complications and who have not achieved clinically meaningful weight loss with lifestyle intervention alone" [7].
Step Therapy: Which Drugs Come First?
Step therapy protocols require a member to try and fail a less expensive medication before the plan will authorize the preferred agent. For Wegovy, step therapy often means a documented trial of one of the following:
- Orlistat 120 mg three times daily (generic available; FDA-approved for obesity) [8]
- Phentermine-topiramate extended-release (Qsymia) 7.5/46 mg to 15/92 mg
- Naltrexone-bupropion extended-release (Contrave) 8/90 mg titrating to 32/360 mg daily
Each of these agents has a substantially lower list price than semaglutide 2.4 mg, which carries a 2024 wholesale acquisition cost of approximately $1,349 per month. The STEP-1 trial (N=1,961) showed semaglutide 2.4 mg produced 14.9% mean body weight loss at 68 weeks versus 2.4% with placebo (P<0.001) [9]. By comparison, orlistat produces roughly 3% greater weight loss than placebo at one year [10], and naltrexone-bupropion produces approximately 5% greater weight loss than placebo at 56 weeks in the COR-I trial (N=1,742) [11]. These efficacy differences are clinically meaningful and can be cited in a prior authorization appeal.
How to Get Wegovy Covered: A Step-by-Step Approach
Getting prior authorization approved, or overturning a denial, requires a systematic approach. The following framework is based on standard managed care appeal procedures and the clinical evidence base.
Step 1: Confirm Your Plan Type
Call Affinity member services at (800) 223-7242 or log into the member portal at affinityplan.org to confirm whether your specific plan tier includes anti-obesity medications. Ask for the specific formulary tier and prior authorization criteria document for semaglutide 2.4 mg (NDC prefix 00169).
Step 2: Build the Clinical File
Your prescriber should compile a clinical file that includes:
- Current BMI with date of measurement
- List of weight-related comorbidities with ICD-10 codes (E11.9 for type 2 diabetes, I10 for hypertension, E78.5 for hyperlipidemia)
- Documentation of prior diet, exercise, or behavioral intervention and duration
- Documentation of any prior anti-obesity medication trials, doses used, duration, and reason for discontinuation or inadequate response
- A letter of medical necessity that cites the STEP-1 trial data and the AHA/ACC obesity guideline recommendation [7]
Step 3: Submit the Prior Authorization
Affinity accepts prior authorization requests by fax or through the Availity provider portal. Turnaround for standard PA is typically 3 business days; urgent requests require 24 hours under New York State managed care regulations. If the plan denies the request, it must provide a written explanation.
Step 4: Appeal a Denial
New York State law gives members the right to an internal appeal, followed by an external appeal through the New York State Department of Financial Services if the internal appeal is denied. External appeals for denials involving FDA-approved drugs are decided by an independent organization (IMO). In a 2022 analysis of specialty drug appeals across New York managed care plans, approximately 50% of external appeals for anti-obesity medications were overturned when supported by peer-reviewed clinical evidence. Cite the SELECT cardiovascular outcomes data [3] and the 2024 FDA label update [4] if the denial references lack of cardiovascular benefit.
Out-of-Pocket Costs and Savings Options
Even with commercial coverage, Wegovy sits on a specialty tier with a typical 25% to 33% coinsurance, translating to $337 to $445 per month at the 2024 WAC. Several cost-reduction pathways exist.
Novo Nordisk Savings Card
Novo Nordisk offers a savings card for commercially insured patients that may reduce the monthly cost to as low as $0 for eligible members. This card is not available to federal or state government program beneficiaries, including Medicaid, Medicare Part D, and Essential Plan members [12]. Patients can enroll at WegovyHCP.com or through their pharmacy.
NovoCare Patient Assistance Program
Uninsured or underinsured patients with a household income at or below 400% of the federal poverty level may qualify for NovoCare, Novo Nordisk's patient assistance program, which can provide Wegovy at no cost. Documentation of income and a completed enrollment form are required. The prescriber must submit the form on the patient's behalf.
340B Covered Entities
Federally Qualified Health Centers (FQHCs) and other 340B-covered entities purchase covered outpatient drugs at significantly reduced prices. Affinity Medicaid members who receive care at an FQHC may be able to access semaglutide at 340B pricing for an approved indication (type 2 diabetes via Ozempic) even when the commercial route is unavailable [13].
Clinical Evidence Supporting Coverage: What to Cite in an Appeal
Insurance appeals succeed when they speak in the plan's language: clinical evidence, FDA labeling, and guideline support. The following data points are the strongest cards in the deck.
Weight Loss Outcomes
The STEP-1 trial enrolled 1,961 adults with BMI of 30 or above, or 27 or above with at least one comorbidity, and no diabetes. Participants randomized to semaglutide 2.4 mg once weekly lost a mean of 14.9% of body weight at 68 weeks, compared to 2.4% in the placebo group (P<0.001) [9]. The STEP-4 trial (N=803) further showed that participants who discontinued semaglutide regained two-thirds of their lost weight within one year [14], supporting the argument for long-term coverage rather than a limited trial authorization.
Cardiovascular Risk Reduction
The SELECT trial (N=17,604) showed a 20% reduction in major adverse cardiovascular events (MACE) with semaglutide 2.4 mg over a median follow-up of 34.2 months in patients with existing cardiovascular disease, BMI of 27 or above, and no diabetes (hazard ratio 0.80; 95% CI 0.72 to 0.90; P<0.001) [3]. The FDA approved the cardiovascular risk reduction indication in March 2024 [4]. Plans that deny Wegovy purely on the basis of it being a "lifestyle drug" face a harder argument after this label expansion.
Guideline Endorsement
The Endocrine Society's 2023 obesity pharmacotherapy guideline recommends GLP-1 receptor agonists as first-line pharmacotherapy for obesity in appropriate candidates [15]. The American Diabetes Association's 2024 Standards of Care similarly recommend semaglutide for weight management in adults with type 2 diabetes and obesity [16]. Both documents can be submitted as supporting literature in an appeal.
What to Do If Affinity Denies Coverage Completely
A flat denial is not the end of the road. New York's managed care appeal process has several layers, and the clinical evidence for semaglutide is strong enough to support a credible challenge.
Internal Appeal
Submit a written internal appeal within 60 days of the denial notice. Include the clinical file described in Step 2 above, a letter of medical necessity from the prescriber, and copies of the relevant trial publications (STEP-1 [9], SELECT [3]).
External Appeal
If Affinity upholds the denial internally, request an external appeal through the New York State Department of Financial Services (DFS) within 45 days. External appeals for medical necessity denials are reviewed by independent medical experts. For a drug with an FDA-approved cardiovascular indication, the legal standard for denial is high.
Peer-to-Peer Review
Before the formal appeal, ask the prescriber to request a peer-to-peer review with Affinity's medical director. This informal call sometimes resolves denials without a full appeal. The prescriber should reference the STEP-1 weight loss data [9] and the AHA/ACC guideline statement [7] during this conversation.
