Because alirocumab is a large protein degraded by proteolysis rather than cytochrome P450 enzymes, traditional drug-drug interaction screens (CYP inhibition, P-glycoprotein transport) do not apply [2]. No randomized controlled trial, case series, or pharmacovigilance signal in the FDA Adverse Event Reporting System (FAERS) links reishi mushroom to adverse outcomes in patients taking any PCSK9 inhibitor. Reishi Mushroom Pharmacology: What Clinicians Should Know
Reishi (Ganoderma lucidum) contains over 400 bioactive compounds. The two classes most relevant to an alirocumab interaction assessment are triterpenoids (ganoderic acids) and polysaccharides (beta-glucans).
Antiplatelet and Anticoagulant Properties
Ganoderic acids inhibit platelet aggregation by interfering with thromboxane A2 synthesis. A 2016 in vitro study published in Phytochemistry demonstrated that ganoderic acid S suppressed ADP-induced platelet aggregation by 36% at 100 micromol/L [3]. A separate open-label pilot (N=10) found that 1.5 g/day of reishi extract for 4 weeks prolonged bleeding time by a mean of 1.2 minutes from baseline [4]. These effects are modest in healthy volunteers, but they become clinically meaningful when patients are already on anticoagulants or antiplatelets, a common co-prescription in the ASCVD population that receives alirocumab.
Immune Modulation and Beta-Glucans
Reishi beta-glucans stimulate macrophage phagocytosis and shift cytokine production toward a Th1-dominant profile (increased IL-2, IFN-gamma; decreased IL-4, IL-10) [5]. Patients concurrently taking warfarin (INR target 2.0 to 3.0) or dual antiplatelet therapy (DAPT) after recent percutaneous coronary intervention carry the highest marginal risk from adding reishi. Lower-Risk Patients
A patient taking alirocumab as monotherapy for heterozygous familial hypercholesterolemia, with no anticoagulant or antiplatelet use and no autoimmune history, faces minimal added risk from a standard reishi dose (1 to 1.5 g/day of dried extract). The interaction concern is real but dose-dependent and context-dependent.
Monitoring Plan If You Take Both
If a patient and their prescriber decide to continue both agents, a structured monitoring protocol reduces the chance of missing an adverse signal.
Baseline Labs Before Adding Reishi
Draw a CBC with differential and platelet count, a comprehensive metabolic panel (CMP) including ALT and AST, and a fasting lipid panel. These establish the pre-supplement reference range.
Follow-Up at 8 to 12 Weeks
Repeat the same panel. Key thresholds to flag:
- Platelet count drop of more than 15% from baseline
- ALT or AST rise above 2x the upper limit of normal (reishi hepatotoxicity has been reported in at least three published case reports at doses exceeding 3 g/day) [9]

Step 2: Review Your Full Medication List
The interaction concern scales with the total number of agents affecting hemostasis. If you take aspirin 81 mg, clopidogrel 75 mg, and reishi 1.5 g daily alongside Praluent, the antiplatelet "stack" is four agents deep. Your prescriber may recommend discontinuing the reishi or, alternatively, dropping one of the pharmaceutical antiplatelets if your ASCVD risk profile permits.
Step 3: Lab Check
Even if you feel fine, a CBC and LFT draw confirms there is no subclinical platelet depression or hepatic injury. Reishi-associated hepatotoxicity is rare but documented. A 2017 analysis by ConsumerLab found that 6 of 12 tested reishi products contained less than 50% of the beta-glucan content stated on the label. Patients should look for products bearing a USP Verified, NSF International, or ConsumerLab seal, which confirms identity, potency, and absence of heavy-metal contamination.
The Bottom Line for Clinicians and Patients
No published evidence demonstrates a dangerous interaction between reishi mushroom and alirocumab. The theoretical risks are pharmacodynamic: additive antiplatelet effects in patients already on antithrombotic therapy, immune modulation at the injection site, and rare hepatotoxicity from reishi itself. These risks are manageable with disclosure, baseline labs, and follow-up monitoring at 8 to 12 weeks. Patients taking DAPT or anticoagulants alongside Praluent should avoid reishi until discussing it with their prescriber. For patients on alirocumab monotherapy without bleeding risk factors, a standard-dose reishi product (1 to 1.5 g/day, third-party tested) represents a low but not zero-risk addition.
The Endocrine Society and the American College of Cardiology have not issued formal guidance on supplement co-administration with PCSK9 inhibitors [10]. Until that changes, shared decision-making between patient and prescriber, supported by lab monitoring, remains the standard of care.
