Pharmacologic Basis for Titration

The magnitude of this effect is dose-dependent and varies between agents due to differences in PDE5 selectivity, pharmacokinetics, and tissue distribution. Tadalafil's 17.5-hour half-life, for example, permits once-daily dosing, while sildenafil's 3- to 5-hour half-life favors on-demand use [1].
Why One Size Does Not Fit All
Individual variation in CYP3A4 and CYP2C9 metabolism, baseline endothelial function, and autonomic tone means that the same milligram dose produces widely different plasma concentrations across patients. A 2005 meta-analysis of 27 sildenafil trials (N=6,659) reported efficacy rates of 56% at 25 mg, 77% at 50 mg, and 84% at 100 mg, confirming a clear dose-response curve [2]. These data make systematic titration, rather than fixed-dose prescribing, the standard of care.
PDE5 Selectivity and Cross-Reactivity
Sildenafil and vardenafil show measurable inhibition of PDE6 (retinal) and PDE11 (skeletal muscle, testes), which explains visual disturbances and myalgias at higher doses. Tadalafil is more selective for PDE11 than PDE6, producing fewer visual complaints but occasional back pain. Avanafil is the most PDE5-selective agent in the class. ED Titration: The Standard Algorithm
The goal of ED titration is to find the lowest effective dose that produces erections sufficient for satisfactory intercourse with acceptable side effects. The American Urological Association (AUA) 2018 guidelines recommend starting at mid-range doses and adjusting based on at least four to six attempts [4].
Step 1: Starting Dose Selection
Begin with sildenafil 50 mg, tadalafil 10 mg, vardenafil 10 mg, or avanafil 100 mg. Reduce the starting dose by half in patients aged 65 and older, those with hepatic impairment (Child-Pugh B), renal impairment (CrCl <30 mL/min for sildenafil), or concomitant CYP3A4 inhibitors. The FDA label for sildenafil specifies a 25 mg starting dose with ritonavir and a maximum of 25 mg per 48 hours [5].
Step 2: Adequate Trial Period
Instruct patients to attempt intercourse at least four to eight times at a given dose before concluding it has failed. A single unsuccessful attempt does not constitute treatment failure. Counsel on proper timing (sildenafil and vardenafil 30 to 60 minutes before, avanafil 15 to 30 minutes before, tadalafil 1 to 2 hours before or daily), the need for sexual stimulation, and avoidance of high-fat meals with sildenafil and vardenafil [6].
Step 3: Dose Escalation
If the initial dose is ineffective after an adequate trial, escalate to the next available strength:

If maximum dose fails after a further four to eight attempts, consider switching to a different PDE5 inhibitor before declaring class failure. Cross-switching succeeds in roughly 30% to 50% of initial non-responders, likely due to pharmacokinetic and selectivity differences between agents [7].
Step 4: Dose De-Escalation
Once a patient achieves stable efficacy, attempt a step-down to the next lower dose at a follow-up visit (typically 3 months). Patients with improving endothelial function (e.g., after weight loss, statin initiation, smoking cessation) may maintain efficacy at reduced doses. This is not tapering in the pharmacologic sense but dose optimization.
Daily Tadalafil: Titration and Tapering for ED and BPH
Tadalafil 5 mg daily is FDA-approved for both ED and BPH/LUTS. The LVHJ study (N=1,500) demonstrated that daily tadalafil 5 mg improved International Prostate Symptom Score (IPSS) by a mean of 4.8 points versus 2.2 for placebo at 12 weeks [8].
Initiating Daily Therapy
Start at 2.5 mg daily for 2 to 4 weeks, then escalate to 5 mg daily if tolerated but insufficiently effective. For patients with CrCl 30 to 50 mL/min, begin and remain at 2.5 mg daily. Daily dosing reaches steady state in approximately 5 days given the 17.5-hour half-life [1].
Tapering Off Daily Tadalafil
Abrupt discontinuation of daily tadalafil is pharmacologically safe (no withdrawal syndrome), but patients often report a subjective "rebound" worsening of LUTS or ED symptoms. A practical approach:
- Reduce from 5 mg daily to 2.5 mg daily for 2 to 4 weeks.
- Then switch to 2.5 mg every other day for 1 to 2 weeks.
- Discontinue, or transition to on-demand tadalafil 10 mg as needed.
This step-down prevents the abrupt loss of tonic cGMP-mediated smooth muscle relaxation in the prostatic urethra and penile vasculature. No randomized trial has formally studied taper protocols, but expert consensus from the European Association of Urology (EAU) supports gradual withdrawal when patients have used daily tadalafil for more than 3 months [9].
Titration in Pulmonary Arterial Hypertension
PDE5 inhibitors carry a distinct indication in PAH, and titration here follows different principles than ED dosing. Hemodynamic monitoring, right heart catheterization data, and functional class drive adjustments.
Sildenafil for PAH (Revatio)
The SUPER-1 trial (N=278) established sildenafil 20 mg TID as the FDA-approved PAH dose, showing a 45-meter improvement in 6-minute walk distance (6MWD) versus placebo at 12 weeks [10]. Although the 80 mg TID arm showed additional benefit, the FDA approved only 20 mg TID due to the adverse-effect profile.
In clinical practice, many PAH specialists titrate above 20 mg TID (off-label) when response is suboptimal. A common protocol starts at 20 mg TID for 4 weeks, assesses 6MWD and WHO functional class, then increases to 40 mg TID if the patient remains in functional class III or IV. The maximum studied dose is 80 mg TID [10].
Tadalafil for PAH (Adcirca)
The PHIRST trial (N=405) demonstrated that tadalafil 40 mg once daily improved 6MWD by 33 meters versus placebo [11]. The once-daily regimen simplifies adherence compared to sildenafil TID.
Start tadalafil at 20 mg daily for 1 to 2 weeks, then increase to 40 mg daily. In patients on bosentan (a CYP3A4 inducer), begin at 20 mg and uptitrate to 40 mg. With ritonavir or other strong CYP3A4 inhibitors, avoid co-administration entirely or limit to 20 mg daily under close monitoring.
PAH Discontinuation: Never Stop Abruptly
Sudden withdrawal of PDE5 inhibitors in PAH can trigger rebound pulmonary vasoconstriction, acute right heart failure, and death. The 2022 European Society of Cardiology (ESC)/European Respiratory Society (ERS) pulmonary hypertension guidelines state: "Discontinuation of PAH-targeted therapy should only be considered in expert centres and must be performed gradually" [12].
A representative taper for sildenafil 20 mg TID:
- Reduce to 20 mg BID for 2 weeks with hemodynamic monitoring.

Drug Interaction-Driven Dose Modifications
PDE5 inhibitors are CYP3A4 substrates. Co-administration with CYP3A4 inhibitors or inducers requires mandatory dose adjustments, not optional ones.
Nitrates: Absolute Contraindication
Concurrent use of any PDE5 inhibitor with organic nitrates (nitroglycerin, isosorbide mononitrate/dinitrate, amyl nitrite) is contraindicated due to synergistic hypotension. Allow a washout period of at least 24 hours after sildenafil or vardenafil, and at least 48 hours after tadalafil, before administering nitrates [5].
Alpha-Blockers
The interaction produces additive hypotension. Tamsulosin 0.4 mg is the best-studied alpha-blocker for co-prescribing due to its uroselective alpha-1A profile. Start the PDE5 inhibitor at the lowest dose (sildenafil 25 mg, tadalafil 5 mg on-demand, vardenafil 5 mg). Ensure the patient has been stable on the alpha-blocker for at least 1 week before introducing the PDE5 inhibitor. Avoid dosing both agents within 4 hours of each other [13].
Strong CYP3A4 Inhibitors
Ketoconazole, itraconazole, ritonavir, and clarithromycin all significantly raise PDE5 inhibitor plasma levels. Specific adjustments per FDA labeling:
- Sildenafil: maximum 25 mg per 48 hours with ritonavir [5]
- Tadalafil on-demand: maximum 10 mg per 72 hours with ketoconazole or ritonavir
- Vardenafil: maximum 2.5 mg per 24 hours with ketoconazole 400 mg or ritonavir [14]
- Avanafil: maximum 50 mg per 24 hours with moderate CYP3A4 inhibitors; contraindicated with strong CYP3A4 inhibitors [15]
Special Populations
Dose selection in special populations is not a separate algorithm. It is the same algorithm with a different starting point.
Hepatic Impairment
Child-Pugh A: standard starting dose. Child-Pugh B: reduce starting dose by 50% for sildenafil, vardenafil, and avanafil; tadalafil on-demand should not exceed 10 mg. Child-Pugh C: sildenafil, vardenafil, and avanafil are not recommended; tadalafil lacks sufficient data [5][14][15].
Renal Impairment
For CrCl <30 mL/min, start sildenafil at 25 mg. Tadalafil on-demand should begin at 5 mg, with a maximum of 10 mg no more frequently than every 48 hours if CrCl is <30. Tadalafil daily dosing is not recommended when CrCl is <30 mL/min. Vardenafil and avanafil require no renal dose adjustment per their labels [14][15].
Older Adults
Age alone does not mandate a lower starting dose for tadalafil or avanafil. Sildenafil 25 mg and vardenafil 5 mg are recommended starting doses in patients over 65 based on pharmacokinetic data showing 40% higher plasma concentrations in this group [5][14]. Co-prescribe cautiously with antihypertensives and monitor for orthostatic hypotension at each dose change.
Monitoring and Follow-Up
Titration without follow-up is incomplete prescribing.
ED Follow-Up Schedule
Schedule the first follow-up 4 to 6 weeks after initiating therapy (allowing time for the adequate trial period). Assess efficacy with the International Index of Erectile Function (IIEF-5) or the Sexual Encounter Profile (SEP) questions 2 and 3 ("Were you able to insert your penis?" and "Did your erection last long enough?"). The IIEF-5 threshold for "no ED" is a score of 22 or above [16].
What to Check at Each Visit
- Blood pressure (seated and standing if on antihypertensives or alpha-blockers)
- Adverse effects: headache, flushing, nasal congestion, dyspepsia, visual changes, myalgia/back pain (tadalafil)
- Medication reconciliation: new nitrates, alpha-blockers, CYP3A4 inhibitors, or recreational nitrite ("poppers") use
- Cardiovascular risk: PDE5 inhibitors are not first-line treatments for cardiovascular disease; the Princeton III Consensus recommends exercise stress testing for intermediate-risk patients before prescribing [17]
PAH Monitoring
6MWD at baseline and every 3 to 6 months. NT-proBNP every 3 months. Echocardiography every 6 to 12 months. Right heart catheterization when clinical deterioration is suspected or before dose escalation beyond approved ranges [12].
When Titration Fails: Class Failure vs. Agent Failure
Failure at maximum dose of one PDE5 inhibitor is agent failure. Failure after adequate trials of two or more PDE5 inhibitors (including at least one long-acting and one short-acting agent) constitutes class failure. The AUA guidelines recommend proceeding to intracavernosal injection therapy (alprostadil, trimix), vacuum erection devices, or penile prosthesis evaluation at this point [4].
Before declaring class failure, rule out correctable causes of non-response: improper timing, high-fat meals (sildenafil, vardenafil), insufficient sexual stimulation, performance anxiety, undiagnosed hypogonadism (check morning total testosterone), or severe arteriogenic ED (penile duplex ultrasound showing peak systolic velocity <25 cm/s) [4].
Testosterone replacement in hypogonadal men (total T <300 ng/dL) can restore PDE5 inhibitor responsiveness. A randomized trial by Spitzer et al.
