Within 24 to 72 hours of a Reclast infusion, approximately 32% of patients experience an acute-phase reaction (APR) consisting of fever, myalgia, arthralgia, and headache [1]. The APR is mediated by transient release of pro-inflammatory cytokines (IL-6, TNF-alpha) from peripheral blood gamma-delta T cells [6]. During this window, mild hypotension has been documented.
