How the FDA Classifies Empagliflozin's Adverse Events
The FDA-approved prescribing information for empagliflozin organizes adverse reactions into common, uncommon, and rare categories based on the pooled Phase III development program. Understanding this hierarchy is the fastest way to set accurate patient expectations before the first prescription is written.
What the Prescribing Label States
The current Jardiance label lists genital mycotic infections, urinary tract infections (UTIs), increased urination, and dyslipidemia as adverse reactions with an incidence of at least 2% and exceeding placebo. Volume depletion events (hypotension, syncope, dehydration) appear in a separate warnings section alongside diabetic ketoacidosis, urosepsis, pyelonephritis, and necrotizing fasciitis of the perineum (Fournier gangrene). EMPA-REG OUTCOME as the Primary Safety Dataset
EMPA-REG OUTCOME (N=7,020 adults with T2DM and established cardiovascular disease) remains the largest and most cited randomized controlled trial for empagliflozin safety. At a median follow-up of 3.1 years, the trial showed that empagliflozin reduced the risk of the primary composite MACE endpoint and, critically, generated a prospectively collected adverse-event dataset large enough to detect rare events at a frequency of approximately 1 in 1,000. The trial's safety supplement reported genital mycotic infections in 6.4% of empagliflozin-treated patients versus 1.8% of placebo patients (P<0.001), establishing the most statistically strong signal in the trial program. Volume depletion adverse events occurred in 2.2% versus 1.3% (placebo), driven largely by patients on loop diuretics at baseline. Mild-to-Moderate Adverse Events: Phenotype-Specific Incidence
Most patients experience adverse events that are self-limiting or manageable with minor interventions. The phenotype of the patient, however, changes the probability substantially.
Genital Mycotic Infections
Genital mycotic infections represent the highest-frequency adverse event class for empagliflozin. Glucosuria creates a substrate-rich environment for Candida species in the perineum. A pooled analysis of 12 Phase II/III empagliflozin trials published via the FDA's drug-approval documents confirms this mechanism.
Phenotype-specific rates from the EMPA-REG OUTCOME safety analysis:
- Women with a history of prior vaginal candidiasis: estimated 3-fold higher recurrence risk compared to women without prior infection.
- Uncircumcised men: higher absolute rate of balanitis compared to circumcised men, though the absolute difference in the overall trial was modest (approximately 3% vs. 1%).
- Women overall: approximately 10% incidence across pooled Phase III data.
- Men overall: approximately 3.5 to 4% incidence.

Urinary Tract Infections
The glucosuria hypothesis predicts high UTI rates for all SGLT2 inhibitors, but the clinical trial data for empagliflozin are more nuanced. In EMPA-REG OUTCOME, UTI incidence was 8.1% in the empagliflozin group versus 8.1% in placebo, showing no statistically significant difference. Post-market FAERS data tell a different story for severe UTI phenotypes. Women aged over 65, patients with neurogenic bladder, and those with a history of recurrent UTIs show disproportionate representation in FAERS serious UTI reports for the SGLT2 inhibitor class. Polyuria and Volume Depletion
Empagliflozin's osmotic diuresis effect typically produces a modest 200 to 400 mL per day increase in urine output during the first two weeks, after which compensatory mechanisms attenuate the effect. The clinical problem concentrates in patients who are already volume-depleted at baseline. In EMPA-REG OUTCOME, patients on three or more antihypertensives at baseline had a 3.3% rate of volume depletion adverse events compared to 1.8% in the overall empagliflozin arm. Elderly patients (age 75 and older) warrant extra caution. Serious Adverse Events: Risk Stratification by Phenotype
The events that carry the greatest clinical weight are rare in absolute terms but predictable enough to be preventable with correct patient selection.
Diabetic Ketoacidosis
Empagliflozin-associated diabetic ketoacidosis (DKA) is often euglycemic, meaning blood glucose may be below 250 mg/dL at presentation. This atypical presentation delays diagnosis and worsens outcomes. The FDA issued a Drug Safety Communication on euglycemic DKA for the entire SGLT2 inhibitor class in 2015.
Phenotypes at highest DKA risk with empagliflozin:
- Patients with latent autoimmune diabetes in adults (LADA) or misclassified Type 1 diabetes: Off-label use in insulin-deficient states is the single largest risk factor. A case series in Diabetes Care identified that 70% of SGLT2 inhibitor-associated DKA cases occurred in patients with low or absent endogenous insulin secretion. - Patients on very-low-carbohydrate diets: Carbohydrate restriction reduces insulin secretion and increases glucagon, potentiating ketogenesis. Practitioners should counsel patients to temporarily discontinue empagliflozin at least 3 days before elective surgery or during extended fasting. - Patients with recurrent alcohol use: Alcohol independently promotes ketogenesis. The combination with SGLT2 inhibition carries additive ketogenic risk.

In the pooled EMPA-REG OUTCOME and Phase III development program, adjudicated DKA occurred in 0.1% of empagliflozin-treated patients versus 0.0% of placebo patients. Fournier Gangrene (Necrotizing Fasciitis of the Perineum)
Fournier gangrene is a rare but life-threatening polymicrobial infection of the perineal and genital soft tissues. The FDA added a warning for the entire SGLT2 inhibitor class in August 2018 after reviewing 12 cases in FAERS over approximately 5 years of post-market exposure (estimated tens of millions of patient-years). The estimated background rate of Fournier gangrene in the general diabetic population is approximately 1.6 per 100,000 person-years. Risk phenotypes include:
- Male sex (historical 10:1 male predominance, though SGLT2 inhibitor cases have been more evenly distributed).
- Poorly controlled diabetes (HbA1c above 9%).
- Obesity (BMI above 35 kg/m²).
- Chronic kidney disease stage 3 or above.
- Immunosuppression.
Urosepsis and Pyelonephritis
Unlike lower-tract UTI, upper-tract infections with empagliflozin carry serious morbidity. Renal Adverse Events: A Nuanced Phenotype Story
Empagliflozin produces an initial acute reduction in eGFR of approximately 2 to 4 mL/min/1.73 m² within the first 2 to 4 weeks. This is a hemodynamic, reversible effect related to reduced glomerular hyperfiltration. Over 2 to 3 years, eGFR decline in empagliflozin-treated patients is slower than placebo. The EMPA-KIDNEY Trial
EMPA-KIDNEY (N=6,609 patients with CKD, eGFR 20 to 45 mL/min/1.73 m² or eGFR 45 to 90 with urinary albumin-to-creatinine ratio at least 200 mg/g) tested empagliflozin 10 mg specifically in patients with CKD. The trial was stopped early for efficacy. Empagliflozin reduced the risk of kidney disease progression or cardiovascular death by 28% (hazard ratio 0.72, 95% CI 0.64 to 0.82, P<0.001). Who Faces Acute Kidney Injury Risk
Volume depletion-mediated AKI is the renal adverse event to monitor, especially in:
- Patients on concurrent NSAIDs, ACE inhibitors, and loop diuretics (the "triple whammy" combination).
- Patients with baseline eGFR <45 mL/min/1.73 m².
- Elderly patients with reduced renal reserve.

Cardiovascular and Metabolic Adverse Events
Hypotension and Syncope
The osmotic diuresis and natriuresis from empagliflozin reduce systolic blood pressure by approximately 3 to 5 mmHg in patients with T2DM. This is generally beneficial, but phenotypes at risk for symptomatic hypotension include:
- Patients on three or more antihypertensive agents.
- Patients with autonomic neuropathy.
- Patients with baseline systolic blood pressure below 110 mmHg.

LDL Cholesterol and Hematocrit Changes
Empagliflozin produces a modest increase in LDL cholesterol of approximately 2 to 4 mg/dL and a 2 to 3 percentage-point increase in hematocrit (reflecting hemoconcentration and/or stimulation of erythropoiesis). Neither change is clinically concerning in most patients. Patients with baseline LDL above 190 mg/dL may warrant closer monitoring. Heart Failure Phenotype: Adverse Events in EMPEROR-Reduced and EMPEROR-Preserved
Heart failure patients represent a growing share of empagliflozin prescriptions. The adverse-event profile in this population differs meaningfully from the T2DM-only population.
EMPEROR-Reduced Safety Data
EMPEROR-Reduced (N=3,730 patients with HFrEF, ejection fraction at or below 40%) found that empagliflozin 10 mg reduced the composite of cardiovascular death or hospitalization for heart failure by 25% (HR 0.75, 95% CI 0.65 to 0.86, P<0.001). From a safety standpoint, volume depletion adverse events occurred in 9.0% of empagliflozin patients versus 6.4% of placebo patients. The absolute excess of approximately 2.6 percentage points reflects the already-volume-overloaded baseline state of HFrEF patients. Genital mycotic infections occurred at 1.0% versus 0.3%, lower than T2DM-only populations because many HFrEF patients do not have active T2DM.
EMPEROR-Preserved Safety Data
EMPEROR-Preserved (N=5,988 patients with HFpEF, ejection fraction above 40%) showed empagliflozin reduced the primary composite endpoint by 21% (HR 0.79, 95% CI 0.69 to 0.90, P<0.001). EMPEROR-Preserved results are published in the New England Journal of Medicine.
Adverse events in HFpEF patients closely mirrored EMPEROR-Reduced. Volume depletion was the primary adverse event of clinical concern, reinforcing the recommendation to reassess diuretic dosing when initiating empagliflozin in any heart failure subtype.
Bone Fracture Risk: What the Evidence Shows
Canagliflozin carries an FDA label warning for bone fracture risk. Empagliflozin does not carry this same warning, and the mechanistic basis appears different. Empagliflozin does not inhibit the sodium-phosphate cotransporter NaPi-IIb, which is the proposed mechanism behind canagliflozin's fracture signal. In EMPA-REG OUTCOME, fracture rates were 2.0% in the empagliflozin group versus 2.1% in placebo, showing no excess signal. Elderly patients with pre-existing osteoporosis remain a phenotype to monitor empirically, though current trial data do not support an empagliflozin-specific fracture warning. Hypoglycemia: The Phenotype Where Empagliflozin Is Safer
Empagliflozin's glucose-lowering mechanism is insulin-independent. As a result, hypoglycemia is rare when empagliflozin is used as monotherapy or with metformin. The risk profile changes with concomitant insulin or sulfonylurea use.
In EMPA-REG OUTCOME, adjudicated hypoglycemia requiring assistance occurred in 1.2% of empagliflozin patients on background insulin versus 0.7% without background insulin. Drug Interactions That Amplify Adverse Events
Several common co-medications shift empagliflozin's risk distribution meaningfully.
Diuretics
Loop and thiazide diuretics potentiate volume depletion. In EMPA-REG OUTCOME, volume depletion adverse events were nearly twice as common in patients on baseline diuretics. Adding empagliflozin to these agents increases hypoglycemia risk. NSAIDs
Concurrent NSAID use adds afferent arteriolar constriction to the efferent dilatation already present with empagliflozin-induced volume depletion, creating the condition for hemodynamic AKI. Monitoring Parameters and Clinical Checkpoints
Based on FDA label requirements and trial-derived evidence, the following monitoring schedule applies:
- Before initiation: eGFR, urinalysis, blood pressure, baseline HbA1c to screen for possible T1DM/LADA phenotype. Confirm eGFR is above 20 mL/min/1.73 m² (above 30 for glycemic indication). eGFR thresholds are detailed in the FDA label.
- At 2 to 4 weeks: Recheck eGFR, serum creatinine, and blood pressure. The initial eGFR dip is expected; a fall greater than 30% warrants hold and reassessment.
