What Saxenda Is and Why Coverage Varies

Saxenda is the brand name for liraglutide 3 mg, a GLP-1 receptor agonist injected daily and FDA-approved specifically for chronic weight management in adults with a BMI ≥30 or a BMI ≥27 with at least one weight-related condition such as type 2 diabetes, hypertension, or dyslipidemia. The FDA granted this approval in December 2014 based on the SCALE Obesity and Prediabetes trial program. [1]

Anthem BlueCross BlueShield is not a single insurer. It is a collective of regional plans operating under a licensee agreement with BCBS. Each regional Anthem entity, including Anthem Health Plans of Virginia, Anthem Blue Cross of California, and Empire BlueCross in New York, negotiates its own formulary independently. That means a member in Ohio may have different Saxenda coverage terms than a member in Georgia, even if both say "Anthem" on their insurance card. [2]

How Formularies Work for Obesity Drugs

A formulary is the list of drugs a plan agrees to cover at negotiated rates. The Affordable Care Act does not require commercial insurers to cover obesity medications, so each plan decides independently whether to include them at all. [3] For plans that do include Saxenda, it almost always lands on a specialty or Tier 4 position, which carries the highest member cost-sharing.

Why Employer Plans Add Another Layer

Many Anthem members receive coverage through an employer-sponsored self-funded plan. Under the Employee Retirement Income Security Act (ERISA), self-funded employers design their own benefit packages and are not required to follow state insurance mandates. [4] An employer can instruct Anthem to exclude obesity medications entirely, or to add step-therapy requirements that the standard Anthem commercial formulary does not impose. Always check the Summary Plan Description provided by your HR department, not just the general Anthem formulary published online.

Prior Authorization Requirements for Saxenda on Anthem Plans

Prior authorization (PA) is the process by which Anthem requires your prescriber to submit clinical documentation before the plan will pay for Saxenda. Nearly every Anthem plan with Saxenda on formulary requires PA. [5]

Standard Clinical Criteria Anthem Typically Requires

Anthem's medical policy documents for anti-obesity pharmacotherapy generally require all of the following before approving Saxenda:

  • Documented BMI ≥30 kg/m², or BMI ≥27 kg/m² with at least one qualifying comorbidity (type 2 diabetes, hypertension, obstructive sleep apnea, or dyslipidemia)
  • Evidence of a structured, supervised diet and exercise program lasting at least six months, with documented lack of sufficient response
  • Absence of contraindications including personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2, consistent with the Saxenda prescribing information [6]
  • No concurrent use of another GLP-1 receptor agonist or insulin secretagogue in many plan versions

Step Therapy: What It Means in Practice

Step therapy requires trying a less costly drug first. For weight management, Anthem plans frequently require a documented trial of orlistat (Xenical, Alli) at therapeutic doses before Saxenda qualifies. Some plans accept a trial of phentermine/topiramate extended-release (Qsymia) or naltrexone/bupropion (Contrave) instead. [7] A prescriber must document that the step drug was tried and either failed to produce adequate weight loss or caused intolerable adverse effects. The FDA defines adequate response to anti-obesity medication as at least 5% weight loss after 12 weeks at full therapeutic dose. [8]

How to Submit a Prior Authorization

  1. Your prescriber or their office submits a PA request through Anthem's online portal or by fax using the applicable drug-specific form.
  2. Anthem reviews against its clinical criteria, typically within 3 business days for non-urgent requests and 24 hours for urgent ones (federal standard under 45 CFR 147.136).
  3. If approved, coverage begins immediately and typically runs for 12 months before renewal is required.
  4. If denied, Anthem must issue a written denial with the specific clinical reason, the criteria used, and instructions for appeal. [9]

How to Check Your Specific Anthem Plan for Saxenda Coverage

General guidance only goes so far. These four steps give you a definitive answer for your own plan.

Step 1: Use the Anthem Formulary Search Tool

Log into your Anthem member account at anthem.com and manage to the pharmacy benefits or drug list section. Search for "liraglutide" or "Saxenda." The tool will display the tier, any PA or step-therapy flags, and your estimated cost-sharing for a 30-day supply based on your deductible status.

Step 2: Call Anthem Member Services

The member services number is printed on the back of your insurance card. Ask the representative specifically: (a) Is Saxenda on my formulary? (b) Is prior authorization required? (c) Is step therapy required, and if so, what drugs qualify? (d) What is my cost-sharing after deductible? Document the call date, representative name, and reference number.

Step 3: Request the Medical Policy Document

Anthem publishes medical policy documents that detail the clinical criteria for PA approval. Ask member services for the specific medical policy number governing anti-obesity agents. Reading the raw criteria lets your prescriber tailor the PA submission precisely to what Anthem requires, which reduces back-and-forth delays. [10]

Step 4: Have Your Prescriber Call the PA Hotline

Prescribers can call Anthem's pharmacy prior authorization line directly. A physician-to-physician peer review call can sometimes resolve a borderline case more quickly than a paper submission alone.

What Saxenda Costs Without or With Anthem Coverage

Without insurance, Saxenda costs approximately $1,400 to $1,600 for a 30-day supply in the United States as of early 2025, according to pharmacy pricing data. [11] That translates to roughly $16,800 to $19,200 per year out of pocket.

Cost With Anthem Coverage

When Saxenda is covered, the member cost varies by plan design. On a typical Anthem commercial plan with a specialty tier copay of $100 to $150 per fill, a patient who has met their deductible might pay $100 to $150 per month. Before the deductible is met, cost-sharing can be substantially higher. Some high-deductible health plans (HDHPs) require members to pay the full negotiated price until the deductible, sometimes $3,000 to $5,000, is satisfied.

Novo Nordisk Savings Card

Novo Nordisk, the manufacturer, offers a savings card for commercially insured patients that can reduce out-of-pocket cost to as low as $25 per fill for eligible patients. Patients on government insurance programs (Medicare, Medicaid, TRICARE) do not qualify. Eligibility and terms are published directly by Novo Nordisk and can change annually. [12]

Medicare and Medicaid Considerations

Traditional Medicare Part D historically excluded coverage of weight-loss medications under the Social Security Act's drug exclusion rules. The Treat and Reduce Obesity Act, introduced in multiple Congressional sessions, has aimed to change this, but as of early 2025, coverage for Saxenda specifically under Part D remains limited to plans that have voluntarily chosen to include it, and only where it is prescribed for type 2 diabetes management under the liraglutide 1.8 mg (Victoza) indication rather than the 3 mg obesity indication. Medicaid coverage varies by state. [13]

The SCALE Trial Data: Why Saxenda Is Worth Pursuing Coverage For

Understanding the clinical evidence helps frame the coverage conversation with your insurer.

The SCALE Obesity and Prediabetes trial (N=3,731) randomized adults with BMI ≥30 or BMI ≥27 with dyslipidemia or hypertension to liraglutide 3 mg or placebo, combined with diet and exercise counseling, for 56 weeks. Liraglutide 3 mg produced a mean weight loss of 8.4% versus 2.8% with placebo (P<0.001). More strikingly, 63.2% of liraglutide-treated participants lost at least 5% of body weight compared with 27.1% on placebo. [14]

A secondary SCALE trial in patients with type 2 diabetes (N=846) showed 6.0% mean weight loss with liraglutide 3 mg versus 2.0% with placebo at 56 weeks. [15]

The American Association of Clinical Endocrinology (AACE) 2016 obesity guidelines state: "Anti-obesity medications are recommended as an adjunct to lifestyle therapy for patients with BMI ≥30 or BMI ≥27 with complications, when lifestyle therapy alone does not achieve clinically meaningful weight loss." [16] This guideline language maps directly onto the PA criteria Anthem uses, which means your prescriber can cite AACE guidance explicitly in the clinical justification letter.

Cardiovascular Context

The LEADER trial (N=9,340) evaluated liraglutide 1.8 mg (Victoza) in patients with type 2 diabetes and high cardiovascular risk and found a significant reduction in major adverse cardiovascular events (MACE) compared to placebo: hazard ratio 0.87 (95% CI 0.78 to 0.97, P<0.001 for non-inferiority, P=0.01 for superiority). [17] While this trial used the 1.8 mg dose rather than the 3 mg obesity dose, the cardiovascular safety data informs formulary committees reviewing liraglutide as a drug class.

What to Do If Anthem Denies Saxenda Coverage

A denial is not the end of the process. Federal law guarantees appeal rights for most commercial plans.

Internal Appeal

Under the ACA and ERISA, you have the right to an internal appeal within 180 days of receiving a denial notice. Your prescriber submits additional clinical documentation directly addressing the denial reason. [18] The most effective appeal letters include:

  • The patient's complete weight history with BMI measurements over at least 12 months
  • Documentation of prior dietary interventions and outcomes
  • A step-therapy failure letter if applicable
  • A direct quotation of the specific Anthem medical policy criteria alongside evidence that the patient meets each criterion
  • Supporting peer-reviewed literature, including the SCALE trial results

External Appeal

If Anthem upholds its denial after an internal appeal, you may request an external independent review organization (IRO) review. For fully insured plans, state law governs this right. For self-funded ERISA plans, federal external review rules apply. The IRO's decision is binding on Anthem. [19]

Expedited Appeal for Urgent Cases

If the standard timeline would seriously jeopardize your health or your ability to regain maximum function, you may request an expedited internal appeal. Anthem must respond within 72 hours. [20]

Alternatives Anthem May Cover More Readily

If Saxenda faces repeated coverage barriers, several alternative anti-obesity medications may have more favorable formulary positioning on Anthem plans.

Qsymia (Phentermine/Topiramate ER)

Qsymia is a Schedule IV controlled substance combining phentermine and topiramate extended-release. The CONQUER trial (N=2,487) demonstrated 9.8% mean weight loss at 56 weeks with the highest dose (15 mg/92 mg) versus 1.2% with placebo (P<0.001). [21] Some Anthem commercial plans place Qsymia on a preferred brand tier, making it more accessible than Saxenda.

Contrave (Naltrexone/Bupropion SR)

Contrave combines naltrexone and bupropion in an extended-release formulation. The COR-I trial (N=1,742) showed 6.1% mean weight loss at 56 weeks with the 32 mg/360 mg dose versus 1.3% with placebo. [22] It is an oral medication, which some patients prefer, and it is frequently placed on a lower cost-sharing tier than injectable GLP-1 agents.

Wegovy (Semaglutide 2.4 mg)

Wegovy (semaglutide 2.4 mg weekly) received FDA approval in June 2021. The STEP-1 trial (N=1,961) showed 14.9% mean weight loss at 68 weeks versus 2.4% with placebo (P<0.001). [23] Anthem coverage for Wegovy is also variable and plan-dependent, but some 2024 and 2025 plan years have added Wegovy while removing or restricting Saxenda, particularly after the SELECT cardiovascular outcomes trial in 2023 demonstrated a 20% reduction in MACE with semaglutide 2.4 mg in patients without diabetes. [24]

Orlistat (Xenical)

Orlistat 120 mg is the only FDA-approved weight-loss drug available as a generic and is almost universally covered on Anthem formularies at a low tier. It produces modest weight loss (approximately 3 to 4 kg more than placebo at 12 months in meta-analyses) and is primarily useful as a step-therapy qualifier to document inadequate response before seeking PA for Saxenda or Wegovy. [25]

Practical Prescriber Tips for Getting Saxenda Approved

A well-prepared PA submission dramatically improves approval rates. These steps reflect the clinical documentation standards outlined in Anthem's own medical policy framework.

Document the Six-Month Lifestyle Intervention

Anthem requires evidence of supervised diet and exercise. Ideally, this comes from a registered dietitian, a structured weight management program such as the Diabetes Prevention Program (DPP), or office visit notes with recorded BMI measurements and counseling documentation at each visit over at least six months. [26]

Frame the Comorbidity Carefully

If the patient has a BMI between 27 and 30, the comorbidity documentation is what unlocks coverage eligibility. A formal diagnosis in the problem list, an ICD-10 code on the PA form, and recent lab values or blood pressure readings confirming active disease strengthen the submission considerably.

Use the Anthem Clinical Criteria Language

Pull Anthem's published medical policy document for anti-obesity agents and write the justification letter to mirror its exact criteria language. If the policy says "six-month supervised dietary program," use exactly that phrase in the letter. Vague letters that describe the patient's weight history without referencing the specific policy criteria are the most common reason PAs fail on the first submission.

Request a Peer-to-Peer Review

If a PA is denied by a pharmacy technician or non-specialist reviewer, the prescriber can request a peer-to-peer call with an Anthem medical director. This conversation allows the prescriber to address clinical nuances that a form-based submission cannot capture. Studies of PA outcomes across specialties have found that peer-to-peer review reverses initial denials in 30 to 75% of cases depending on the drug category and plan. [27]

Frequently asked questions

Does Anthem cover Saxenda for weight loss?
Anthem may cover Saxenda for weight loss on some commercial plan designs, but coverage is not guaranteed. Most Anthem plans place Saxenda on a specialty or non-preferred tier and require prior authorization. Coverage depends on your specific employer contract, plan year, and state. Check your Summary Plan Description or call the member services number on your insurance card for a definitive answer.
What BMI do I need for Anthem to cover Saxenda?
Anthem's standard clinical criteria for Saxenda prior authorization require a BMI of 30 kg/m² or higher, or a BMI of 27 kg/m² or higher with at least one weight-related comorbidity such as type 2 diabetes, hypertension, or dyslipidemia. These thresholds match the FDA-approved labeling for Saxenda.
Does Anthem require step therapy before covering Saxenda?
Many Anthem plans require step therapy, meaning you must try and document an inadequate response to at least one other weight-loss medication, commonly orlistat, before Saxenda will be approved. The specific step-therapy requirement varies by plan. Your prescriber should request Anthem's medical policy document to confirm the exact step required for your plan.
How much does Saxenda cost with Anthem insurance?
With Anthem coverage and after meeting your deductible, Saxenda typically costs $100 to $150 per month on plans with a specialty tier copay. Before the deductible is met, cost-sharing can be substantially higher. Without insurance, Saxenda retails for approximately $1,400 to $1,600 for a 30-day supply.
What happens if Anthem denies my Saxenda prior authorization?
You have the right to appeal. First, your prescriber can request an internal appeal within 180 days of the denial by submitting additional clinical documentation addressing the specific denial reason. If the internal appeal is upheld, you can request an independent external review. An expedited appeal, with a 72-hour response, is available if your health is at serious risk.
Does Medicare cover Saxenda?
Traditional Medicare Part D generally does not cover Saxenda (liraglutide 3 mg) for weight management because of a longstanding exclusion of weight-loss drugs from Part D coverage. Some Part D plans have voluntarily added Wegovy (semaglutide 2.4 mg) following its cardiovascular outcomes data, but coverage of Saxenda specifically under Medicare remains very limited as of early 2025.
Is Saxenda better than Wegovy for insurance coverage?
Neither drug consistently has better insurance coverage. Anthem's formulary decisions change annually, and some 2024 to 2025 plan years have moved toward covering Wegovy over Saxenda due to the SELECT trial cardiovascular outcomes data for semaglutide. Always check your current plan formulary rather than assuming one drug is more accessible than the other.
Can I use a manufacturer coupon for Saxenda with Anthem?
Novo Nordisk offers a savings card that can reduce Saxenda out-of-pocket costs for commercially insured patients to as low as $25 per fill. The card cannot be used with government insurance programs including Medicare, Medicaid, and TRICARE. Terms change annually, so confirm current eligibility directly with Novo Nordisk.
Does Anthem Medicaid cover Saxenda?
Anthem Medicaid coverage for Saxenda varies by state. Each state's Medicaid program sets its own drug coverage policies, and obesity medications are frequently excluded or heavily restricted. Contact your state's Anthem Medicaid plan directly to confirm whether Saxenda is on the formulary for your specific program.
What alternative weight loss drugs does Anthem cover more easily?
Orlistat (Xenical) is the most widely covered weight-loss medication on Anthem plans because a generic is available. Contrave (naltrexone/bupropion) and Qsymia (phentermine/topiramate ER) are oral medications that some Anthem plans place on preferred tiers. Wegovy (semaglutide 2.4 mg) is increasingly covered on some Anthem commercial plans for 2024 and 2025.
