Immediate-Release vs. Extended-Release Coverage
Generic metformin ER is also a preferred generic on most Anthem plans, though a small number of regional Anthem subsidiaries place it on Tier 2. The practical difference is a copay of $0 to $10 (Tier 1) versus $10 to $25 (Tier 2). Brand-name Glucophage XR is a different story. Because a bioequivalent generic exists, Anthem typically excludes brand Glucophage XR from coverage outright or places it on Tier 4 or 5 unless you obtain prior authorization demonstrating a medically necessary reason the generic formulation cannot be used.

Mark Cuban Cost Plus Drugs (costplusdrugs.com). Metformin HCl 500 mg, 90 tablets costs approximately $4 to $7 at Cost Plus as of early 2025. Cost Plus charges a transparent markup of 15% over manufacturing cost plus a $3 dispensing fee and $5 shipping.
$4 Generics Lists. Walmart, Kroger, and Publix pharmacy programs offer 30-day supplies of generic metformin for $4 and 90-day supplies for $10 without any coupon or insurance required. Eligibility does not depend on insurance status.
State Pharmaceutical Assistance Programs
Several states where Anthem operates Medicaid managed care plans have additional pharmaceutical assistance for patients who fall into coverage gaps. Contact your state's Medicaid office or your Anthem case manager to ask about Extra Help (Low Income Subsidy) programs if you are on a Medicare Part D plan.
Metformin's Clinical Evidence: Why Anthem Covers It
Understanding why Anthem places metformin on Tier 1 requires understanding the drug's evidence base. Anthem formulary committees follow clinical practice guidelines and assess cost-effectiveness.
Type 2 Diabetes Evidence
The UK Prospective Diabetes Study (UKPDS), which enrolled 1,704 overweight patients with newly diagnosed type 2 diabetes, showed that metformin reduced all-cause mortality by 36% and diabetes-related endpoints by 32% compared to conventional diet therapy over a median 10.7-year follow-up (P<0.01 for both comparisons). [1] This landmark trial remains a foundational basis for guideline-driven metformin prescribing.
The 2024 ADA Standards of Medical Care in Diabetes state: "Metformin is recommended as a first-line pharmacological agent for the treatment of type 2 diabetes in most patients, given its established efficacy, safety profile, low cost, and weight-neutral to weight-reducing effects." [2] That explicit guideline language gives Anthem strong grounds to cover the drug at the lowest possible tier.
Metformin lowers HbA1c by approximately 1.0% to 1.5% as monotherapy, a magnitude confirmed across meta-analyses including a Cochrane systematic review (94 trials, N=8,gold) covering head-to-head comparisons with sulfonylureas and thiazolidinediones. [3]
Cardiovascular and Weight Considerations
Unlike sulfonylureas, metformin does not cause hypoglycemia when used as monotherapy. Unlike thiazolidinediones such as pioglitazone, it does not cause fluid retention or increase fracture risk. The Endocrine Society's 2023 clinical practice guideline notes that metformin may produce modest weight reduction of 1 to 3 kg over 12 to 24 weeks in patients with type 2 diabetes, which adds to its value compared to weight-neutral or weight-positive alternatives. The Longevity Angle: TAME and Observational Data
A frequently cited observational analysis published in Aging Cell (2014) examined 78,241 metformin-treated type 2 diabetes patients and 12,222 sulfonylurea-treated patients matched to 78,227 non-diabetic controls. Metformin-treated patients had modestly longer survival than matched non-diabetic controls (hazard ratio 0.85, P<0.001), a finding that sparked intense interest in metformin as a geroprotective drug. [5] The TAME trial is designed to test this hypothesis in a pre-registered, placebo-controlled setting.
Dr. Nir Barzilai, principal investigator of the TAME trial and director of the Institute for Aging Research at Albert Einstein College of Medicine, stated in a 2023 interview with the National Institute on Aging: "Metformin is one of the best candidates we have because it targets multiple aging mechanisms simultaneously, including mTOR, AMPK, and mitochondrial complex I."
Whether that evidence will eventually translate into Anthem coverage for non-diabetic longevity use is an open question. Currently it does not.
Anthem Metformin Coverage for Specific Patient Groups
Patients With Prediabetes
The ADA 2024 Standards of Care state that metformin may be considered for patients with prediabetes (fasting glucose 100 to 125 mg/dL or HbA1c 5.7% to 6.4%), particularly those who are obese, aged <60 years, or have a history of gestational diabetes, with a recommendation grade of B. [2] Anthem coverage for prediabetes is plan-dependent. Some Anthem commercial plans cover metformin for prediabetes with an appropriate ICD-10 diagnosis code (R73.09, "other abnormal glucose"), while others require the prescriber to document a type 2 diabetes diagnosis. Confirm with your plan before the prescription is submitted.
Women With Polycystic Ovary Syndrome (PCOS)
Metformin is used off-label in PCOS to improve insulin sensitivity and restore ovulatory cycles. The Endocrine Society PCOS Guideline recommends metformin for menstrual irregularity in PCOS. [6] Anthem coverage for this indication also varies by plan. Some plans cover it under the metabolic syndrome diagnosis; others classify it as off-label and require PA. Ask your prescriber to document insulin resistance with a fasting insulin level or HOMA-IR calculation to strengthen any PA submission.
Patients on GLP-1 Receptor Agonists
Many patients use metformin alongside semaglutide (Ozempic, Wegovy) or tirzepatide (Mounjaro, Zepbound). Anthem covers metformin at its standard tier regardless of whether you are also prescribed a GLP-1 agent, as the two drugs work through complementary mechanisms and are commonly co-prescribed per ADA guidelines. How to Appeal an Anthem Metformin Coverage Denial
If Anthem denies coverage for metformin under any circumstance, you have legally protected appeal rights under the Affordable Care Act and ERISA.
Internal Appeal
