Quitting nicotine while starting Wegovy: weekly log
Note your quit date, what you smoked or vaped and the aid you chose, then log nicotine use, patch step or varenicline dose, weight, resting heart rate and nausea each week as Wegovy steps up. The patch schedule and the label warning signs are printed alongside.
Where you are starting
Quit date
Cigarettes a day before quitting, or vaping and other nicotine use
Quit aid chosenpatch, gum, lozenge, varenicline, bupropion, none
Resting heart rate and blood pressure before Wegovy
Weight on the quit date
Nicotine patch steps on the NicoDerm CQ label[1]
For more than 10 cigarettes a day. At 10 or fewer a day the label skips Step 1: 14 mg for 6 weeks, then 7 mg for 2 weeks, then stop.
| Step | Patch | Weeks |
|---|---|---|
| Step 1 | 21 mg a day | Weeks 1-6 |
| Step 2 | 14 mg a day | Weeks 7-8 |
| Step 3 | 7 mg a day | Weeks 9-10 |
Week-by-week check
Write the Wegovy dose for each week so nausea or a heart-rate change can be matched to a dose step or to the quit aid.
| Week | Wegovy dose | Cigarettes or nicotine used | Patch step or varenicline dose | Weight | Resting heart rate | Nausea: none, mild, bad |
|---|---|---|---|---|---|---|
Numbers from the sources
- People who quit without medicine gained on average 4.67 kg by 12 months, most of it in the first three months.[2]
- Varenicline 1 mg twice daily: nausea in 30% versus 10% on placebo. If nausea is intolerable, the label says a dose reduction should be considered.[3]
- Wegovy raised resting heart rate by 1 to 4 beats per minute on average compared with placebo; a sustained rise means stopping Wegovy.[4]
Stop and call your healthcare provider for
- New or worse mood or behavior changes on varenicline, such as agitation, depressed mood or suicidal thoughts.[3]
- Irregular heartbeat or palpitations, or signs of too much nicotine (nausea, vomiting, dizziness, weakness, rapid heartbeat) while using the patch.[1]
- A resting heart rate that stays higher than usual on Wegovy.[4]
To ask at the 4-week and 12-week visits
- Which quit aid adds the least nausea while my Wegovy dose is still going up?
- Should I quit first and start Wegovy later, or start both now?
- Is my resting heart rate change from Wegovy, from nicotine, or from both?
- How will we confirm I have quit, and when?
- If I slip back to smoking, should anything about my Wegovy plan change?
