Wegovy break-and-restart worksheet
Use this before the restart visit the page describes: your last injection date, weight since stopping, why you stopped, current medicines and new diagnoses, with the label's restart rule and the trial numbers on regain alongside.

Raise these before any restart
- Pancreatitis, gallbladder trouble or severe stomach symptoms while on Wegovy
- Personal or family history of medullary thyroid cancer or MEN 2
- Pregnancy, or plans to become pregnant
- A new heart, kidney or liver diagnosis since stopping
- New medicines, especially insulin or a sulfonylurea
What the label and the trials say
- Wegovy label: after 2 or more missed weekly doses in a row, dose escalation restarts at a lower dose; your prescriber chooses which.[1]
- Medication Guide: if you miss doses for 2 or more weeks in a row, contact your healthcare provider about how to restart.[1]
- Planning a pregnancy: stop Wegovy at least 2 months beforehand.[1]
- STEP 1 extension: a year after stopping, the semaglutide group had regained 11.6 percentage points, about two-thirds of the weight they lost.[2]
- STEP 4: switching to placebo after 20 weeks meant +6.9% over the next 48 weeks, against −7.9% for those who continued.[3]

Get medical help instead of waiting for the visit
- Severe stomach pain that will not go away, with or without vomiting.[1]
