Fish oil with NMN or NR: total omega-3 and blood-thinner review

Add up the EPA and DHA you get from every source, tick any blood thinner, bleeding disorder, liver problem, surgery or pregnancy that applies, and note any bruising or bleeding. Take it to the prescriber or pharmacist who manages your blood thinner before changing the omega-3 dose.

What the prescription omega-3 labels state

  • Vascepa's label sets the daily dose at 4 grams.[1]
  • In Vascepa's outcomes trial of 8,179 patients, 12% on the drug and 10% on placebo had a bleeding event; the label asks prescribers to monitor people also on anticoagulants or antiplatelet drugs.[1]
  • Lovaza's label also sets 4 grams a day, says bleeding-time prolongation in trials stayed within normal limits, and says people also taking an anticoagulant or antiplatelet drug should be monitored periodically.[2]

On warfarin: bleeding signs its Medication Guide says need a call right away

  • Unusual bruising, meaning bruises that appear without a known cause or grow in size.[3]
  • Bleeding from a cut that takes a long time to stop, nosebleeds or bleeding gums.[3]
  • Red or black stools.[3]

Questions for the prescriber or pharmacist

  1. Is my total EPA plus DHA a day a concern with the blood thinner I take?
  2. Should I pause fish oil before my surgery or procedure, and for how long?
  3. Would a prescription omega-3 suit my triglycerides better than a supplement?
  4. Does adding NMN or NR change anything about my bleeding risk?
