Step 0: Confirm This Is Telogen Effluvium, Not Something Else

Before starting any intervention, rule out competing diagnoses. Telogen effluvium (TE) produces diffuse thinning across the entire scalp, not patches. The patient typically notices increased hair on the pillow, in the shower drain, or on a brush. There is no scalp inflammation, no scarring.

Perform a hair pull test: grasp about 60 hairs between thumb and forefinger near the scalp and slide firmly to the tips. Extracting more than 6 hairs (roughly >10%) is a positive result consistent with active TE. Do this in at least three scalp regions.

Order baseline labs:

If there is patchy loss, exclamation-point hairs, or scalp scarring, refer to dermatology before proceeding. That is not TE.

Step 1: Nutritional Correction (Weeks 1 to 4)

This is the highest-yield intervention because the underlying mechanism is metabolic stress on hair follicles during rapid caloric deficit.

Protein target: 1.2 to 1.5 g per kg of ideal body weight per day. Most patients on Zepbound are eating far less than this because appetite suppression makes it hard to finish meals. Practical approaches:

  • Front-load protein at breakfast and lunch (30 g per meal minimum)
  • Use whey or casein protein shakes to close the gap when whole-food intake is difficult
  • Track intake for 5 to 7 days using a food diary so the target is not theoretical

Micronutrient supplementation based on labs:

  • Ferritin <30 ng/mL: Ferrous bisglycinate 25 to 50 mg daily (better absorbed, less GI upset than ferrous sulfate, which matters on a GLP-1)
  • Zinc <70 mcg/dL: Zinc picolinate 30 mg daily for 8 weeks, then recheck
  • Vitamin D <30 ng/mL: Cholecalciferol 4,000 to 5,000 IU daily until replete, then maintenance at 2,000 IU
  • Biotin: 2,500 to 5,000 mcg daily. Evidence is limited for biotin in the absence of deficiency, but the safety profile is strong. Warn patients that biotin interferes with troponin and thyroid assays, so they must disclose use before any bloodwork.

What success looks like at 4 weeks: The pull test count drops. The patient is meeting protein targets. Lab deficiencies are trending toward correction. Shedding may not have visibly decreased yet, and that is expected. TE has a lag.

Step 2: Rate-of-Loss Assessment (Weeks 4 to 8)

If nutritional correction alone is insufficient, the rate of weight loss itself may be the primary driver. The SURMOUNT-1 data showed that hair loss incidence increased at higher tirzepatide doses (5.7% at 15 mg vs. 4.0% at 5 mg), correlating with faster weight reduction.

Benchmark: Weight loss exceeding 1.5% of body weight per week is a recognized trigger for TE in bariatric surgery literature. Calculate the patient's average weekly rate of loss over the past 8 to 12 weeks.

If the rate exceeds this threshold and shedding is distressing to the patient:

  • Dose hold or stepdown. Rather than discontinuing Zepbound entirely, step back to the previous tolerated dose (e.g., 15 mg to 10 mg, or 10 mg to 7.5 mg). This slows weight loss while preserving metabolic benefit.
  • Caloric floor. Ensure total daily intake is not dropping below 1,200 kcal for women or 1,500 kcal for men. Patients on GLP-1 agonists sometimes eat 800 to 900 kcal without realizing it.
  • Recheck in 4 weeks. If the rate of loss moderates to <1% per week and nutritional targets are being met, the follicular stress signal should begin to resolve.

What success looks like at 8 weeks: Weekly weight loss rate is <1% of body weight. Protein and micronutrient targets are consistently met. The pull test may still be positive, but the count should be trending down. Patients need to understand that hair has a 3-month lag between trigger resolution and visible improvement.

Step 3: Adjunctive Therapies (Months 3 to 6)

If shedding remains active after 8 to 12 weeks of optimized nutrition and a moderated rate of loss, consider adding topical or procedural interventions. These do not treat the cause (metabolic stress) but can support follicle recovery.

Topical minoxidil 5% (foam preferred): Applied once daily to affected areas. Minoxidil prolongs anagen phase and increases follicular blood flow. In TE specifically, it can accelerate recovery by 4 to 8 weeks. Set expectations: initial shedding of miniaturized hairs in weeks 2 to 4 is normal and not a sign of worsening.

Low-level laser therapy (LLLT): FDA-cleared devices (helmet or comb form) used 3 times per week for 15 to 25 minutes. Evidence supports modest benefit in promoting anagen re-entry. This is a reasonable option for patients who prefer non-pharmacologic approaches.

Platelet-rich plasma (PRP) injections: Emerging data in androgenetic alopecia and some TE cases. Not first-line. Consider only if minoxidil is not tolerated or the patient requests it. Cost is a barrier (typically $500 to $1,500 per session, not covered by insurance).

What to avoid: Finasteride and dutasteride are 5-alpha reductase inhibitors indicated for androgenetic alopecia. They have no role in TE caused by weight loss. Prescribing them here adds side effect risk without addressing the actual problem.

Step 4: Escalation and Dermatology Referral

Refer to dermatology if any of these criteria are met:

  • Shedding persists beyond 6 months despite Steps 1 through 3
  • Pull test remains strongly positive (>10%) at month 6
  • New pattern emerges: patchy loss, bitemporal recession, or frontal thinning inconsistent with TE
  • Scalp biopsy is being considered to differentiate TE from early androgenetic alopecia or alopecia areata
  • Patient distress is significant and affecting treatment adherence

The dermatologist can perform trichoscopy (dermoscopic scalp exam) and, if needed, a 4 mm punch biopsy. The biopsy can quantify the telogen-to-anagen ratio. A ratio exceeding 25% telogen follicles confirms active TE.

When to consider stopping Zepbound for hair loss: Almost never as a first response. In the SURMOUNT trials, no participants discontinued tirzepatide due to alopecia. Hair loss from TE is fully reversible once the metabolic trigger resolves. Dose reduction (Step 2) is the appropriate lever. Discontinuation should only be discussed if the patient finds the hair loss intolerable despite all interventions, and only after a shared decision-making conversation that weighs the metabolic benefits of continued treatment.

What Recovery Looks Like

TE recovery follows a predictable arc once the trigger is controlled:

  • Months 1 to 2 post-correction: Shedding rate decreases. Pull test count drops below 6.
  • Months 3 to 4: Short new-growth hairs (1 to 2 cm) become visible, especially at the hairline and part line. These may initially feel coarser or have a slightly different texture.
  • Months 6 to 12: Density normalizes. Full-length regrowth replaces the shed hairs. Most patients report cosmetically acceptable recovery by month 9.

Patients who lost >20% of body weight may experience a longer recovery window (up to 12 to 18 months) because the metabolic trigger was more sustained. This is not a treatment failure. It is the biology of hair cycling.
