How L-Theanine Works in the Body
L-theanine crosses the blood-brain barrier within 30 to 60 minutes of oral ingestion. Once inside the central nervous system, it increases alpha-wave activity on EEG, raises GABA and glycine levels, and partially antagonises glutamate at NMDA receptors. A randomised, placebo-controlled crossover trial by Nobre et al. (2008, N=35) showed that a single 50 mg dose significantly increased alpha-band power bilaterally across occipital and parietal regions (P<0.05) without causing sedation. [1]
How Tadalafil Works in the Body
Tadalafil selectively inhibits phosphodiesterase type 5 (PDE5), preventing the breakdown of cyclic GMP in vascular smooth muscle. The resulting vasodilation increases penile blood flow during sexual stimulation and relaxes smooth muscle in the prostate and bladder neck. The FDA-approved label documents mean blood-pressure reductions of 1 to 3 mmHg systolic and 0.5 to 1 mmHg diastolic in healthy volunteers at the 20 mg dose. [2] Tadalafil's plasma half-life of approximately 17.5 hours is considerably longer than that of sildenafil (approximately 4 hours) or vardenafil (approximately 4 to 5 hours), meaning any interaction has a wider window in which it could appear. [2]
Is There a Known Drug-Supplement Interaction Between L-Theanine and Tadalafil?
No formal pharmacokinetic interaction has been published. The concern worth evaluating is pharmacodynamic: both agents can lower blood pressure, and an additive effect could theoretically cause symptomatic hypotension.
Pharmacokinetic Assessment
Tadalafil is metabolised primarily by CYP3A4. L-theanine is not a CYP3A4 inducer or inhibitor at doses used in supplements; it is absorbed as an intact amino acid and undergoes hydrolysis to glutamate and ethylamine in the gut and kidney. A 2017 review in Food Chemistry documented that theanine bioavailability is approximately 56% after oral dosing and that it does not meaningfully alter hepatic cytochrome P450 enzyme activity. [3] Because the two compounds use entirely different metabolic pathways, a pharmacokinetic interaction is not expected.
Pharmacodynamic Assessment
This is where close attention is warranted. A double-blind crossover study by Yoto et al. (2012, N=14) found that 200 mg L-theanine attenuated caffeine-induced systolic blood pressure elevation by roughly 7 mmHg at 60 minutes post-dose. [4] L-theanine appears to modulate sympathetic tone, which could add to tadalafil's mild vasodilatory effect. In normotensive adults, the combined BP reduction is likely to stay within a clinically safe range. In adults with a baseline systolic pressure already <100 mmHg, or in those taking alpha-blockers (doxazosin, tamsulosin) alongside tadalafil, the margin shrinks considerably.
The FDA label for tadalafil already warns that co-administration with alpha-blockers or antihypertensives may produce additive hypotension. [2] L-theanine's vasodilatory effect is weaker than either of those drug classes, but the principle of additive physiology still applies.
What the Research Actually Shows About L-Theanine and Blood Pressure
Several controlled trials have measured L-theanine's direct effect on resting blood pressure.
Resting Blood Pressure Evidence
A randomised controlled trial by Dodd et al. (2015, N=48, published in Nutritional Neuroscience) found that 200 mg L-theanine reduced resting systolic blood pressure by approximately 3.5 mmHg compared to placebo in high-stress-response participants (P<0.05). [5] An earlier crossover study in the Journal of Physiological Anthropology (Kimura et al., 2007, N=12) showed a statistically significant reduction in heart rate and salivary immunoglobulin A in response to an acute stressor, consistent with parasympathetic promotion. [6]
Relevance to the Tadalafil User
Tadalafil at its ceiling dose of 20 mg reduces systolic BP by approximately 1 to 3 mmHg at rest. [2] Adding L-theanine at 200 mg may contribute a further 3 to 4 mmHg reduction in people who are stress-responsive. In a healthy adult with a systolic pressure of 120 mmHg, a combined reduction of 6 to 7 mmHg is not concerning. In someone whose systolic pressure runs 95 to 100 mmHg before treatment, it could push them into symptomatic territory.
Neither the ACC/AHA hypertension guidelines nor the American Urological Association BPH guidelines specifically address L-theanine co-administration with PDE5 inhibitors, which reflects the absence of case reports or trial data showing clinical harm rather than deliberate omission. [7]
Mechanism Summary: Why This Interaction Is Pharmacodynamic, Not Pharmacokinetic

[8] The compound is GRAS (Generally Recognised As Safe) under 21 CFR 182.20 standards, though the FDA has not issued a formal GRAS notice specifically for L-theanine as a standalone supplement. [9]
No Reported Cases of Tadalafil-Theanine Adverse Events
A search of the FDA Adverse Event Reporting System (FAERS) database and published case report literature returns no documented serious adverse events specifically attributed to the combination of tadalafil and L-theanine. Absence of reported harm is not proof of safety, but it does place the interaction in a lower-priority risk category relative to, for example, tadalafil combined with nitrates (which is absolutely contraindicated) [2] or tadalafil combined with potent CYP3A4 inhibitors such as ketoconazole or ritonavir, which can increase tadalafil AUC by up to 124%. [2]
Caffeine as a Confounding Variable
Many people take L-theanine specifically alongside caffeine, the combination marketed as "smart caffeine." This introduces a second variable when tadalafil is also in the picture.
The Caffeine-Tadalafil Interaction
Caffeine is a mild stimulant that acutely raises blood pressure and heart rate. Used alone, it partially counteracts tadalafil's vasodilatory action through adenosine receptor antagonism and mild sympathetic activation. A 2015 review in the British Journal of Clinical Pharmacology noted that caffeine consumption (3 mg/kg) can blunt nitric-oxide-mediated vasodilation by approximately 30% transiently. [10] Whether that extends to PDE5-inhibitor-mediated vasodilation has not been formally tested, but the mechanistic direction suggests caffeine might reduce, not amplify, tadalafil's efficacy.
Where L-Theanine Fits In
L-theanine counteracts caffeine's blood-pressure-elevating and anxiety-inducing effects, as demonstrated in the Yoto et al. Crossover trial mentioned above. [4] When all three are present (tadalafil + caffeine + L-theanine), the net vascular effect is difficult to predict precisely, but the clinical direction is toward modest vasodilation with attenuated sympathetic tone. For most users, this is the intended outcome. Monitoring blood pressure informally at home with a validated device for the first two to three weeks of combined use is a practical way to stay informed.
Who Should Exercise Extra Caution
Patients on Alpha-Blockers
The FDA label for tadalafil lists alpha-blocker co-administration as a specific warning requiring clinical judgment. [2] Tamsulosin 0.4 mg combined with tadalafil 10 mg or 20 mg produced no significant additive hypotension in trial data, but doxazosin 4 mg combined with tadalafil 20 mg produced mean maximum systolic BP decreases of 7 to 9 mmHg beyond tadalafil alone. [2] Adding L-theanine into that regimen may push the total reduction further.
Patients With Cardiovascular Conditions
The Princeton Consensus Panel III (2012) guidelines on sexual activity and cardiovascular risk categorised men into low, intermediate, and high risk for sexual activity. The American College of Cardiology endorsed the view that PDE5 inhibitors are safe in low-risk patients but require careful evaluation in intermediate or high-risk groups. [11] L-theanine's mild BP effect does not change the core risk stratification, but it is worth disclosing to your cardiologist if you fall into the intermediate or high-risk category.
Patients With Baseline Hypotension
Anyone with a resting systolic pressure consistently <100 mmHg should discuss both L-theanine and tadalafil with their prescriber before combining them. The combined additive effect, while modest in normotensive individuals, could produce symptomatic dizziness or near-syncope when the starting point is already low.
Practical Dosing and Timing Guidance
No dose-separation window is required between L-theanine and tadalafil based on current evidence, since the interaction is pharmacodynamic rather than pharmacokinetic. There is no absorption competition and no shared metabolic pathway that would necessitate staggered timing.
For Daily-Dose Tadalafil (2.5 to 5 mg)
The low-dose daily regimen produces steady-state plasma concentrations after approximately five days. Blood pressure effects at these doses are at the lower end of the 1 to 3 mmHg range. L-theanine taken at 100 to 200 mg once or twice daily is very unlikely to produce a clinically significant additive effect in normotensive adults.
For On-Demand Tadalafil (10 to 20 mg)
On-demand dosing produces peak plasma levels at approximately two hours post-dose and meaningful PDE5 inhibition for up to 36 hours. If you take L-theanine for acute anxiety around the time of sexual activity, the pharmacodynamic overlap occurs at peak tadalafil levels. Taking 100 mg L-theanine rather than 200 to 400 mg during this window is a reasonable conservative approach until you have established your individual blood pressure response.
Monitoring Protocol
Check your blood pressure 90 to 120 minutes after the first co-administration (this coincides with L-theanine's peak CNS effect and is within tadalafil's rising plasma concentration phase).
