They do not. Dulaglutide is a GLP-1 receptor agonist composed of two modified GLP-1 analogue sequences fused to an Fc fragment of human IgG4. It is degraded by general proteolytic pathways, not by cytochrome P450 enzymes or P-glycoprotein transporters [2]. The FDA label for Trulicity states explicitly that "dulaglutide is not expected to cause drug-drug interactions through CYP enzymes or transporters" [2].

Tadalafil, on the other hand, is metabolized primarily by CYP3A4, with a minor contribution from CYP2C9 [3]. Its major circulating metabolite, methylcatechol glucuronide, is pharmacologically inactive against PDE5 at therapeutic concentrations. Drugs that inhibit or induce CYP3A4 (ketoconazole, rifampin, ritonavir) do change tadalafil exposure meaningfully. Dulaglutide does neither.

No published phase I or phase II interaction study has paired these two molecules directly. That absence itself is informative. The FDA requires interaction studies when mechanistic overlap exists. The lack of a mandated study reflects confidence that no CYP-mediated, transporter-mediated, or protein-binding interaction is expected.

The Real Concern: Additive Blood-Pressure Lowering

The pharmacodynamic interaction is where clinical attention belongs. Both drugs reduce systolic blood pressure through independent mechanisms.

Dulaglutide promotes natriuresis and improves endothelial function. In the AWARD-1 trial (N=978), dulaglutide 1.5 mg reduced systolic BP by 2.8 mmHg relative to placebo over 26 weeks [4]. The REWIND cardiovascular outcomes trial (N=9,901) confirmed a sustained SBP reduction of approximately 1.7 mmHg over a median 5.4-year follow-up [5].

Tadalafil inhibits PDE5 in vascular smooth muscle, producing vasodilation. The FDA label reports mean SBP decreases of 1.6 mmHg (10 mg) and 4.7 mmHg (20 mg) in healthy volunteers [3]. In men already taking one or two antihypertensives, the additional SBP drop with tadalafil 20 mg averaged 3.2 mmHg in a pooled analysis of 17 placebo-controlled trials [6].

When stacked, a combined SBP reduction of 5 to 10 mmHg is plausible. For most patients on stable antihypertensive regimens, that magnitude is tolerable. For patients with autonomic neuropathy (common in longstanding diabetes), baseline orthostatic hypotension, or concomitant alpha-blocker use, the additive effect can produce dizziness, presyncope, or falls.

The clinical decision here is not binary. A risk-stratification approach works best: measure seated and standing blood pressure before co-prescribing, repeat at 2 weeks and 6 weeks, and counsel the patient to take tadalafil from a seated position during the first month of overlap.

Gastroparesis and Absorption Timing

GLP-1 receptor agonists slow gastric emptying. This is the mechanism behind their nausea side effect and part of their glycemic benefit (blunting postprandial glucose spikes by delaying nutrient delivery to the small intestine). A scintigraphy study in healthy volunteers given a single 1.5 mg dose of dulaglutide showed a 23% reduction in gastric emptying rate at one hour postmeal, though the effect attenuated by four hours [7].

Tadalafil reaches peak plasma concentration (Tmax) at approximately 2 hours after oral dosing in the fasted state [3]. In the presence of slowed gastric motility, that Tmax could shift to 2.5 to 3.5 hours. The clinical relevance is modest for daily-dose tadalafil (2.5 or 5 mg), where steady-state concentrations minimize the importance of any single Tmax shift. For on-demand tadalafil (10 or 20 mg), the delay may matter: patients should plan dosing 30 to 60 minutes earlier than they otherwise would.

This absorption delay does not change the total amount of tadalafil absorbed (AUC). It changes only the timing of peak effect. No dose adjustment is required for this reason alone.

Patients Taking Nitrates: The Absolute Contraindication

Both drug labels carry the same hard stop. Tadalafil is contraindicated with any form of organic nitrate (nitroglycerin, isosorbide mononitrate, isosorbide dinitrate, amyl nitrite) because PDE5 inhibition potentiates the nitric-oxide-mediated vasodilation that nitrates produce [3]. The result can be profound, life-threatening hypotension.

Dulaglutide itself does not interact with nitrates. But the clinical scenario matters: a patient on Trulicity, tadalafil, and a nitrate is carrying triple-layered BP-lowering risk. The Endocrine Society and the AUA both recommend documenting nitrate status before any PDE5 inhibitor prescription in diabetic men [8]. If a patient uses even sublingual nitroglycerin PRN for angina, tadalafil is off the table regardless of GLP-1 status.

The 48-hour washout rule applies here. After a 20 mg tadalafil dose, plasma levels remain high enough to potentiate nitrate effects for at least 48 hours given its 17.5-hour half-life [3].

Renal and Hepatic Dosing Adjustments When Co-Prescribed

Type 2 diabetes is the leading cause of chronic kidney disease in the United States, affecting approximately 40% of adults with diabetes over time [9]. Both drugs require renal-function awareness.

Dulaglutide does not require dose adjustment based on renal function. The AWARD-7 trial (N=577) specifically enrolled patients with moderate-to-severe CKD (eGFR 15 to 59 mL/min/1.73 m²) and demonstrated that dulaglutide 1.5 mg slowed eGFR decline compared with insulin glargine over 52 weeks [10].

Tadalafil does need renal adjustment. For daily use, the FDA label recommends a starting dose of 2.5 mg in patients with creatinine clearance 30 to 50 mL/min, and the drug is not recommended for daily use below 30 mL/min. For on-demand use in CrCl <30, the maximum recommended dose is 5 mg no more than once every 72 hours [3].

Hepatic impairment follows a similar pattern. Dulaglutide has no hepatic dose modification. Tadalafil exposure increases in moderate hepatic impairment (Child-Pugh B), and the drug should not exceed 10 mg on-demand in that population. It has not been studied in severe hepatic impairment (Child-Pugh C) and should be avoided [3].

What About Other GLP-1 Agonists and PDE5 Inhibitors?

The interaction profile described here applies broadly across the GLP-1 receptor agonist class (semaglutide, liraglutide, tirzepatide) and the PDE5 inhibitor class (sildenafil, vardenafil, avanafil). None of the GLP-1 agonists use CYP metabolism. All PDE5 inhibitors are CYP3A4 substrates to varying degrees. The additive blood-pressure concern is consistent across pairings.

One difference worth noting: tirzepatide (Mounjaro/Zepbound), a dual GIP/GLP-1 agonist, produces a larger SBP reduction than dulaglutide. In the SURMOUNT-1 trial (N=2,539), tirzepatide 15 mg reduced SBP by 7.4 mmHg vs. 1.3 mmHg with placebo at 72 weeks [11]. The additive BP-lowering potential with tadalafil is therefore greater with tirzepatide than with dulaglutide, and monitoring should be proportionally more attentive.

Sildenafil and vardenafil have shorter half-lives (4 to 5 hours and 4 to 6 hours, respectively) compared with tadalafil's 17.5 hours. This means the window of additive hypotension risk is shorter with those agents. For patients with marginal BP, switching from tadalafil to sildenafil PRN may reduce sustained overlap.

Monitoring Protocol for Co-Prescription

A practical monitoring framework for clinicians co-prescribing dulaglutide and tadalafil:

Baseline (before adding the second drug): Record seated and standing BP. Document nitrate use, alpha-blocker use, and baseline autonomic symptoms. Check eGFR and hepatic function.

Week 2: Repeat orthostatic vitals. Ask about dizziness, lightheadedness, or near-syncope. If standing SBP drops more than 20 mmHg or standing SBP falls below 90 mmHg, hold tadalafil and reassess antihypertensive regimen.

Week 6: Repeat vitals. If stable, move to routine visit schedule. If the patient has started or changed antihypertensives during this interval, restart the 2-week check.

Ongoing: At every diabetes follow-up (typically quarterly), confirm nitrate status and BP stability. Counsel patients that alcohol, dehydration, or adding new antihypertensives can re-expose them to hypotension risk even after a stable co-prescription period.

Sexual Dysfunction in Diabetes: The Broader Clinical Picture

ED in type 2 diabetes is not purely vascular. Neuropathic, hormonal, and psychological components all contribute. A 2019 meta-analysis in The Journal of Sexual Medicine (14 studies, N=3,167) found that PDE5 inhibitors improved IIEF-EF domain scores by a mean of 6.6 points in diabetic men, compared with 8.2 points in nondiabetic men [12]. The effect is real but attenuated.

GLP-1 agonists may independently improve erectile function through weight loss, improved endothelial function, and reduced systemic inflammation. A post-hoc analysis of STEP-5 (semaglutide 2.4 mg, N=304) found a 2.3-point improvement in IIEF-EF scores at 104 weeks in the treatment arm versus placebo, though this was not a prespecified endpoint [13]. Whether dulaglutide produces a similar benefit remains unconfirmed by randomized data.

The clinical takeaway: prescribing tadalafil alongside dulaglutide addresses the immediate symptom while dulaglutide may contribute to longer-term vascular and metabolic improvements that partially restore native erectile function over 12 to 24 months.

Patient Counseling Points

Patients should hear five specific things when starting this combination:

  1. Take tadalafil while seated, especially during the first two weeks. Stand up slowly.
  2. If you use on-demand tadalafil rather than daily dosing, take it 30 to 60 minutes earlier than you might expect because Trulicity can slow absorption.
  3. Never combine tadalafil with any nitrate, including recreational poppers (amyl nitrite). This applies regardless of Trulicity use.
  4. Report any episodes of dizziness, blurred vision, or fainting to your prescriber within 24 hours.
